How to get the documents.. double click on each box to open each document, save, then print. (NB: Print out all three documents)
(Hard copies of these three documents are also available at the shop front counter)
Please note, the application form cannot be filled in and sent online... it must be printed out, completed  then either posted or handed in to the Op Shop by 4pm on  6th March 2026.

There will be a workshop held to assist in the completion of applications and for any further information needed about the process. The workshop will be held at Kendall Services Club on Wednesday 18th February 2026 at 10am.
 

*ACQUITTAL FORM.
The Op Shop Grants Sub Committee asks that you complete an evaluation of your project if you receive a grant to complete it.

 

Note: if you received a grant in 2025  and wish to make a further application in 2026, please ensure that before the closing date for this year's applications (06/03/26) you have returned the 2025 acquittal form. You will be ineligible for a grant in 2026 if this acquittal form is not returned.
