Please note, the application form cannot be filled in and sent online... it must be printed, then either posted or handed in to the Op Shop by 4pm on  9th February 2024.

*EVALUATION.
The Op Shop Committee asks that you complete an evaluation of your project,
if you receive a Grant to complete it. 
Please note, the evaluation form cannot be filled in and sent online... it must be printed, then either posted or handed in to the Op Shop just before the Grants Presentation ceremony in  2025.
